Chest PA (Erect)
Patient erect, facing the vertical Bucky / chest stand, chin raised and resting on top of IR.
How to perform this view
- 01
Remove all metal, jewellery and clothing from waist up; give gown.
- 02
Stand patient facing IR with feet apart for stability.
- 03
Raise chin and place on top of IR; adjust IR so top is 4-5 cm above shoulders.
- 04
Place backs of hands on hips, elbows forward; roll shoulders forward against IR.
- 05
Ensure no rotation - both shoulders equidistant from IR.
- 06
Centre CR to T7 (inferior angle of scapula), collimate to lung fields.
- 07
Expose on second full inspiration.
Anatomical & patient position
Patient: Patient erect, facing the vertical Bucky / chest stand, chin raised and resting on top of IR.
Part: Mid-sagittal plane (MSP) perpendicular and centred to IR; shoulders rolled forward, backs of hands on hips to move scapulae out of lung fields.
Central ray & centring point
Horizontal, perpendicular to IR, centred at T7 (18-20 cm below vertebra prominens).
Exposure factors
kVp
110-125 kVp
mAs
2-4 mAs (AEC: both lateral chambers)
SID
180 cm (72 in) to minimise heart magnification
IR size
35 x 43 cm (14x17 in), portrait or landscape for broad chests
Grid
Yes - high-ratio grid / Bucky (or no grid for small patients)
Respiration
Suspend respiration on second full inspiration
Clinical indications
- Pneumonia, consolidation and infection
- Pneumothorax and pleural effusion
- Cardiomegaly, heart failure
- Lung masses, nodules, TB screening
- Post-procedure line/tube check
- Pre-operative and occupational screening
Contraindications & precautions
- Pregnancy (relative) - confirm LMP / 10-day rule, justify exposure and shield where possible
- Haemodynamically unstable or unable-to-stand patient (perform supine/AP alternative instead)
- Severe trauma with suspected spinal injury (do supine AP)
Structures shown & evaluation criteria
- Both lungs from apices to costophrenic angles
- Heart, mediastinum, trachea
- 10 posterior ribs above diaphragm
- Sternoclavicular joints equidistant from spine (no rotation)
- Scapulae projected outside lung fields
